Forms

The following is a list of forms you may need to fill out, depending on the type of service you are requesting from Pictage. Some Pictage forms are in Microsoft Word and/or .pdf format, and can be downloaded by right-clicking (control-click on Mac) on the links below and selecting "Save Target As" or "Save Link As". Microsoft Word forms can be edited using the Microsoft Word application and e-mailed as attachments.

Support Forms

Pictage’s Customer Experience is committed to providing the best support to all of our clients. To achieve this goal, our Customer Experience Representatives are trained in specific areas and receive ongoing education about the latest Pictage changes.

Photographer Support Form

For general requests and inquiries please complete this online form in full. Make sure you make a selection from the Type: field, based on the reason you are contacting us.

 

Once contacted, please make a note of the representative assisting you as well as the case number, if applicable, in the event that you need future assistance.

Order Cancellation Form

To cancel an order, complete this online form and select Order - Cancellation in the Type: field and a specific reason in the Reason: field.

 

You may request the cancellation of an order on-line within two hours* of when the order was placed. Photographer cancellation requests that come in after the cut-off will be produced, shipped and billed to the photographer at Studio Sample prices. These orders will be marked as "Studio Sample" on each product. Multiple discounts cannot be applied.

 

*This policy has been implemented to provide photographers the fastest possible product turn around times.

Studio Sample Request

To request a studio sample for a specific order, complete this online form and select Studio Sample in the Type: field and a specific reason in the Reason: field.


Photographers may request a Studio Sample of an order, online only before midnight, Pacific Standard Time, on the same day the order is placed. These orders will be marked as "Studio Sample" on each product.

 

  • There is a 50% discount off the base price (additional sides are charged at the standard rate) on WillowBook proof and Coffee Table books.
  • 50% discount applied to entire Sequoia Album order.
  • No sample discount currently available on Fine Art Cards, Promotion Collage Pages and Companion Books.
Print Redo Request Form

If you have received a wrong order, missing print or damaged product, please submit a redo request online using this form. All final print orders will be listed in this form once they have shipped from Pictage. Follow the online instructions on how to place a redo order from your original final print order. Our redo department will review the request and expedite the replacements of your order.

 

If you do not see the order number listed, or if your redo request is not for a final print order, please use the Photographer Support Form and select Order - Redo in the Type: field and a specific reason for the redo request in the Reason: field. Please include the original order number in question in the ORDER ID field, to help expedite your request.

Ready to bind your flush mount album pages? Use these forms to request a beautifully bound album from Leather Craftsmen. Choose from a variety of hand-crafted and genuine leather options to create albums that your clients will treasure for a lifetime. The order form for binding by Leather Craftsmen must be faxed to Pictage immediately after placing the album order in Album Designer. Once you have placed your order, please do not edit your event or album for seven days. However, you may continue to place orders from the event.

Film Processing Worksheet

This worksheet also has detailed instructions on where and how to send your film. Film processing charges will be conveniently added to your monthly statement.

Marketing Forms

Marathon Press Reorder Form

Use this form to reorder marketing products such as the e-mail booklet, event key cards, etc., from Marathon Press.

Guest Email Collection Sheet

Print several copies, and use this form to collect guests' emails at the event.

Guest Pre-Registration E-mail Here's a sample pre-registration e-mail you can forward to your clients. It includes online instructions their guests should follow when pre-registering for an engagement session and/or wedding. Encourage your brides to e-mail it to everyone in their address book. This will let you know how excited the bride is about your service. Additionally, it allows you to market to the friends and family before the event even occurs.

Financial Forms

Pictage Professional Members, to avoid being charged sales tax on products you intend to re-sell to your clients, you must send us a copy of your resale certificate.
